ABSTRACT

OBJECTIVE: To explore the treatment of malignant arrhythmia induced by acute poisoning. METHODS: To analyze the treatment of 11 cases of malignant arrhythmia caused by acute poisoning. The therapeutic effect of malignant arrhythmia treated by drug, electrical cardioversion and detoxification was observed. RESULTS: The arrhythmias in the patients included premature ventricular beats, ventricular tachycardia, ventricular fibrillation, sinus tachycardia, sinus bradycardia, atrial premature beats, and ST-T changes. After treatment with cardioerter drug such as amiodarone, lidocaine, esmolol and other drugs, electric defibrillation, removeing poisons with hemoperfusion and specific antidotes, and multiple organ protection symptomatic treatment, 10 cases were successfully rescued and 1 case died. CONCLUSION: Malignant arrhythmias caused by acute poisoning could be given combined treatment with anti-arrhythmias and relief of etiology.

ABSTRACT

Objective:To study the influence of “Warming Yang and Nourishing Qi”Acupuncture in the expression levels of acetylcholine receptor protein (AchR)in gastrocnemius neuromuscular junction (NMJ)of the rats with experimental autoimmune myasthenia gravis (EAMG),and to clarify the mechanism of “Warming Yang and Nourishing Qi”Acupucture in the treatment myasthenia gravis (MG).Methods:The female Lewis rats were used to establish the EAMG rat models immuned with AchRα1 129-145 polypeptide fragments.30 EAMG rats were selected randomly and divided into model control group, drug control group, and acupuncture treatment group (n=10),and another 10 rats purchased at the same period were used as blank control group. The rats in drug control group were treated with 1 8.5 mg · kg-1 · d-1 pyridostigmine by gavage. And the rats in acupuncture treatment group were treated with irritating by“Warming Yang and Nourishing Qi”Acupuncture. The rats in blank control group and model control group were not treated .The Lennon symptom scores of the rats in various groups were recorded. After treatment, the tissues of gastrocnemius NMJ of the rats were taken, and the AchR protein expression levels in NMJ of the rats in various groups were detected by Western blotting method. Results:Compared with blank control group, after successfully modelling, the Lennon symptom scores of the rats in model control group, drug control group and acupuncture treatment group before treatment were significantly increased (P<0.01).Compared with model control group, the Lennon symptom scores of the rats in drug control group and acupuncture treatment group after two treatment course were significantly increased (P<0.01 ). Compared with blank control group, the AchR protein expression level in gastronemus NMJ of the rats in model control group was significantly decreased (P<0.01). Compared with model control group, the AchR protein expression levels in gastronemus NMJ of the rats in acupuncture treatment group and drug control group after treatment were significantly increased (P<0.01).Conclusion:“Warming Yang and Nourishing Qi”Acupuncture has good therapeutic effect on the EAMG through increasing the NMJ AchR protein expression level of the EAMG rats.

ABSTRACT

Objective:To observe the effect of “Warming Yang and Nourishing Qi” Acupuncture on the acetylcholine receptor (AchR)mRNA expression in neuromuscular junction (NMJ)of the rats with experimental autoimmune myasthenis gravis (EAMG), and to analyze the mechanism of acupuncture treatment for EAMG. Methods:AchRα1 129-145 peptide fragments were used to immune the 60 female Lewis rats to establish the EAMG rat models. 30 EAMG rats were selected randomly and divided into model control group, acupuncture treatment group and drug control group (n = 10).Another 10 SPF- “grade three”female Lewis rats, which were raised together with EAMG rats, were used as blank control group. The rats in model control group and blank control group had no treatment. The rats in acupuncture treatment group were treated with “Warming Yang and Nourishing Qi”Acupuncture. The rats in drug control group were treated with pyridostigmine bromide by gavage. The tissue samples in NMJ of gastrocnemius of the rats were collected, and the expression levels of AChR mRNA were detected with Real-time PCR method; the differences among various groups were analyzed quantitatively. Results:Compared with blank control group, the expression levels ofγ-AChR mRNA andε-AChR mRNA in NMJ of the rats in acupuncture treatment group drug control group and model control group were significantly increased (P <0.01).Compared with model control group,theγ-AChR mRNA andε-AChR mRNA in NMJ of the rats in drug acupuncture treatment group and control group were significantly increased (P < 0.01 ). Conclusion:“Warming Yang and Nourishing Qi”Acupuncture can effectively improve the expression level of AchR mRNA and it is an effective treatment method for myasthenia gravis.

ABSTRACT

Objective To explore the characteristics and drug resistance of pulmonary tuberculosis combined with Stenotrophomonas maltophilia infection. Methods The clinical data of 65 patients with Stenotrophomonas maltophilia infection were analyzed retrospectively. Results It showed that 65 strains of Stenotrophomonas maltophilia mainly came from sputum sample [75.4%(49/65)] and were highly resistant to many kinds of antimicrobial agents,of which the resistance rate to carbapenems,β-lactames was above 60% and the sensitive rate to moxifloxacin,ethoprin-sulfamethoxazole and ticarcillin/clavulanic was above 70%.Conclusion Clinical practices should pay more attention to the high resistance of Stenotrophomonas maltophilia infection, which might result in pulmonary tuberculosis, and prescribe antibiotics rationally according to the drug sensitive test.

ABSTRACT

Objective To investigate if improving or slowing the progression of glucose intolerance might be linked with the reduction of cardiovascular disease(CVD)events and mortality in a Da Qing population with prediabetes. Methods In 1986,577 subjects with impaired glucose tolerance in 33 clinics in Daqing city were randomly assigned to either the control group or one of three lifestyle intervention groups(diet, exercise,or diet plus exercise)to receive a 6 year lifestyle intervention. All the participants were followed for 14 years(1993-2006)after completion of the 6 year active interventions to assess the long-term effect of the interventions. In this post-hoc analysis, the participants were stratified into four subgroups(quartiles)based on their 2 h plasma glucose(2hPG)level after glucose loading at the end of the active intervention,in order to analyze the impact of plasma glucose level on CVD events and mortality. Results During the 20-year follow-up,there were a total of 142 deaths(68 of which were attributed to CVD)and 211 first CVD events(145 strokes and 66 myocardial infarctions). From the highest to the lowest levels of 2hPG in the 4 quartiles, the all-cause mortality(17.8,12.7,10.9,and 9.7/1 000 personyears),CVD mortality(9.1,5.9,6.1,and 4.9/1 1300 person-years)and the incidence of first CVD events(30.4.24.0,18.8,and 19.7/1 000 person-years)showed a clear trend of decline. In multivariate analyses, controlled for age, sex, body mass index, smoking habit, blood pressure, and intervention methods at baseline, the results showed that the 5 mmol/L elevation of 2hPG level after glucose loading in 1992 significantly increased the all-cause mortality(HR 1.335.P=0.005),the incidences offirst CVD events(HR 1.227,P=0.012)and stroke(HR 1.213,P=0.026).Conclusion In pre-diabetes population.if the lifestyle intenrentions are substantially efficacious in improving glucose intolerance, the CVD risk and mortality will be reduced.
